A Resource for Independence Through Integrated Health Care


What, Who, How & Why?


What is it?

MyCare Vermont is a planning grant funded with Federal dollars to integrate the assortment of health care an individual may need.

Who will be eligible?

Vermonters who are elderly, frail, at-risk or chronically ill, and adults with physical disabilities.

How will it work and how is it different?

Choices for Care:

    • Provides person-centered care for individuals 18 years and older.
    • Changes delivery of long-term care services.
    • Provides equal access to nursing facility or home and community based services.

PACE:

    • Provides person-centered care for individuals 55 years and older.
    • Integrates primary/acute and long-term care services through an interdisciplinary team.
    • Operates at a PACE center.

MyCare Vermont will:

    • Provide person-centered care for individuals over the age of 18.
    • Integrate primary/acute and long-term care services through an interdisciplinary team.
    • Operate through community networks.

Why will this change be good for Vermonters?

    • Person-centered care is at the heart of an interdisciplinary team.
    • Lack of integration makes the current system inefficient.
    • An integrated system generates better outcomes.
