Department On-Line Contact Form
The form below allows you to e-mail the Department of Disabilities, Aging and Independent Living (DAIL) with your questions and concerns. The department takes every inquiry and comment very seriously and asks that you fill out the form as completely as possible so your question or problem can be addressed.
E-mails will be responded to within 24 hours from time of receipt. Please note that the State offices are open for business Monday through Friday from 7:45 am to 4:30 pm except for Holidays, and are closed on the weekends.
If you would like to file a complaint, appeal or grievance, please review the procedures in the Policies and Guidelines Section of the DAIL website.
